I had the same problem. Also had problems with other plugins&toolbars not showing (delicious, firebug, …). I uninstalled a bunch of other addons (addblock, noscript, …), and they reappeared. After that i’d recommend installing them back one by one and seeing which one causes the problems…

It does specify this method in Step 2 of the solution.

Another thank you, this has to be one of the longest term valuable help pages on record! One of those irritating things that you never get around to fixing, but that gradually builds up into a frenzy of frustration (rather like Henry VIII and Sir Thomas More!).
